phpcs fixes

• add exclusions for phpcs for third party libraries in phpcs.xml.dist
• fix phpcs issues in javascript files
• replace sc_ prefix with scaffolding_ prefix.
  • phpcs now throws this error for short prefixes:

  • The "sc" prefix is too short. Short prefixes are not unique enough and may cause name collisions with other code.

• Fix or ignore phpcs issues in the php files.
