Thresholds for urbanization #19

Currently, we are re-calculing the threshold value for urbanization in every iteration. Because they are based on the means of all urban and rural cells, this basically means the "bar" for a cell to become urbanized is raised with every iteration.

I'm not sure how to address this without changing our workflow significantly. We would basically need to calculate the country-specific thresholds once and carry them along throughout all simulation steps.
