Add caller to top right corner

Summary of functionality to add
Just like you added the lab in the upper right corner, can you also add the
caller?  So it should look like this, for example:

Lab: Vouloumanos
Caller: Christina Starmans 

Give an example of change in usage of database with new feature (optional).

If you want a form to change, please indicate the way the form looks now
and what you would like the form to look like (optional)?
