Add age range option for scheduling appts

Summary of functionality to add
- Have age range option (as an advanced options) allowing specification of
lower and upper age ranges, which will override any default age ranges for
the selected study
- If age range option is not specified, use default age range associated
with study

New advanced options field 'Custom Age Range' on 'Scheduling Appointments' 
page. If
nothing is input for this field, the default will be the study's age range. If
something is selected (either for lower age or upper age), this will override 
the
study age range. On the subsequent page listing the results, there is text that
indicates this custom age range and the normal default age range for the study 
selected.

Looks good, except the custom age range should default to the age range of the 
study,
so they can just adjust it slightly if they want to.

- page will reload when you select a study when searching 'Scheduling 
Appointments'
- before page reloads, a warning comes on the screen asking user to confirm to
continue (this is because if you have edited the form other than the study 
field,
these edits will be gone)
- when page reloads, age range field will be updated to match that study
