Add headers to tsv file #209

e.g.

#notetype:Chinese
Hanzi   eng pinyin

to the top of the tsv file. Note this still depends on the user having a "Chinese" note type, so we will need to investigate how to create such a thing if it does not exist (may require some tedious instructions..)

addressed in https://github.com/danielt998/HanziToAnki/pull/210 but we still need to add instructions etc
