We are updating the "add new conditions" page on the 526EZ Form from the single-page multiple response pattern to a multi-page multi-step approach to conditions entry.

What's the nature of your initiative and desired outcomes?
In a full implementation of this model, Veterans would answer all questions about a condition before moving on to the next condition. This is in contrast to the current state where Veterans first list all conditions and then are prompted to answer additional questions about each of those conditions

Note: We work on nearly every OCTO product and manage all unauthenticated content on VA.gov, so we will need to prioritize intake requests based on overall workload and VA and OCTO priorities.

Note: For an incremental launch, we use React widgets to display dynamic content. We'll need at least 2 weeks of extra time to coordinate the content for this and work with your engineer.
