What steps will reproduce the problem?
1.visit Demo site: http://pwmdemo.weisberg.net/
2.create a new user
3.enter the validation and input the following information:
ogpalshe@gmail.com
Jeff
Lei
123456
123456
What is the expected output? What do you see instead?
Expect to enter password information, but I see the following error:
"An error has occurred. If this error occurs repeatedly please contact your
helpdesk."
and if I enter password and click it shows error:
PWM 5015

What version of PWM are you using?
Don't know it's your demo site
What ldap directory and version are you using?
