Small tweaks in new rendering #612

This pr addresses:

"New in version 2.14" doesn't render in a box
The red :warning: Symbol isn't showing in
Lists of contents doesn't appear.
the menu headers appear at the top of the documents, in the main container
the menu items appear twice: once before the menu headers, and once after

This doesn't fix the full menu but that need an upstream change. I'm still working on that, and those changes will follow in a next pr.
