It appears as if the largest size radical is the equiv of TeX's variable sized
delimiters which use one or more intermediate elements (vertical bars) to
produce
\sqrt signs of any height.
That would explain why the line is vertical, rather than angled. It would also
explain why it is so much larger than the other radical signs (not meant to be
scaled
to large heights). The gap is explained by there being a small 'L' shaped
connecting
symbol.
Currently no such support for this type of symbol in Mathtex. Fix applied in
r89..
