[APM] Add Host Info to Service Map #195125

Describe the feature: Add the existing host name data to Service Map

Describe a specific use case for the feature: When using Service Maps you only see the name of the service/url/database. It would be beneficial to see the name of the host in the actual map to understand where there service and databases resides from bird's view
