Add functionality to request elevated permissions #174

  1. In sign-in (checkboxes for making edits, uploading data)
  2. In account dashboard (buttons to request access)

These could trigger an email to fathomnet@mbari.org for now, and @kevinsbarnard would handle the permissions updates
