Add flexible tresholds to Flubaroo #60

As of now Flubaroo uses two fixed tresholds :

1- <70% of the maximum grade red flags a student
2- <60% of correct answers red flags a question

But if the quizz is very easy (ex: review of basic concepts at the beginning of 
a term) then 60 or 70% is much too low because you want to be sure that 
(almost) everybody is really confortable with the content. I know in those 
cases i get worried when i see scores below 75-80% correct answers for a 
question, or below 85-90% for a student.

Likewise, 60 or 70% might be too low for a difficult quizz that covers advanced 
material or is more exploratory in nature.

I guees teachers of small classrooms might even want to have individual 
treshold for each student (ex: you will get worried if a straight-A student 
gets 80% accuracy, but you will be very happy if a struggling student manages 
to get 60% !)
