Working on setting up a form to see if this will work for what we need, however I wanted to create it to be similar to what you would see in a spreadsheet where you have a header at the top of the column but I can't figure out how or where you can add something like that, so for example:
Header Header Header (No Field or Input)
Field 1 Option Option
Field 2 Option Option
Field 3 Option Option
Essentially a text field but there would be no need for the input option for text, data or anything.
