Most macros renamed to use new TREEDEC_ prefix #12

Most macros have been renamed to use the prefix TREEDEC. Most include guards have been renamed to the capitalized filename prefixed by TREEDEC. Some files that are not installed still have old names for include guards, in particular the deprecated stuff.
