Refine kin to fhir mapping #49

@cmungall - I took a pass at this.

validated that all of the prior mappings had targets in https://terminology.hl7.org/5.1.0/ValueSet-v3-FamilyMember.html
updated some closeMatches to be narrowMatch or broadMatch
added remaining targets from https://terminology.hl7.org/5.1.0/ValueSet-v3-FamilyMember.html mostly narrowMatch, with just one closeMatch
