WIP: civicpy compatability with molecular profiles #136

This is not yet ready for review/merge. Just pushing what I have so far.

When complete this PR should bring CIViCpy into compatibility with the upcoming release of CIViC that includes a new major entity (Molecular Profiles) as well as schema changes and field renames.
