[PATCH] Group LV2 presets into banks #88

Amsynth has a very large number of presets, which makes a flat menu or combo 
box in hosts nearly unusable.  I've recently added support for preset banks to 
LV2 to resolve this situation.

Patch attached implementing the new vocabulary for amsynth.  Compatibility with 
hosts that do not support banks should be unaffected.
