Added Configuration for Hostname label for Prometheus #22

Hi. I've added a configuration option for which Prometheus label contains the node hostname. This adds support for clusters where Prometheus metric endpoints are targeted by IP instead of hostname, thus the hostname is not in the instance label.

The default configuration remains to use instance.
