We reviewed the parent consent survey form and found out that we wouldn't be able to embed the survey form in the email for parents because we included pictures in the survey form. We created a [new survey copy](moved to trash) where we deleted the pictures out and placed the survey questions as checkboxes in the survey. This way, we are able to embed the survey form in the email for parents. The checkboxes are problematic though since they will mess up the formatting of the survey spreadsheet.

@Annie-Truong Bonnie and Elizabeth had already figured out what the problem was and decided to use the images rather than embed the form in the email, so that's solved now :) I have deleted the new form and spreadsheet we were working on.
