Migrations are a mess #126

Migrations have got in a mess - We need to

1) Reset the migrations and start from scratch 2) Figure out a migration strategy that works with HappeningHQ to avoid them getting in a mess in future

This has now all been reset.

HappeningHQ will submit empty migrations when required to ensure that there are no missing migrations in future.

To ensure compatibility with plugins we will not reset or quash migrations in future.
