from_xml does not create uniform CSV when tags and attributes are missing on some records #42

What steps will reproduce the problem?
1. create an XML file with more than one record.  Have an variable set of tags, 
like <address_line_2> on one of the records.
2. run the csvfix from_xml  command
3. inspect the output file, the columns won't line up

What is the expected output? What do you see instead?
I would expect that there would either A) be a way to specify which tags should 
become columns (or blank) on the output file or that the output file includes a 
column for every tag and attribute in ALL records

What version of the product are you using? On what operating system?
CSVFIX 1.1
Windows XP

Please provide any additional information below.

The documentation says that the command won't convert arbitrary XML files. 
However, I will look at at this. 

In future, please post suggestions to the support groups at 
http://groups.google.com/group/csvfix where they will get more and better 
attention than they will if you post them in the issue tracker.

The originator of this issue has (in personal email) proved to be abusive. No 
further issues will be accepted from him, and any that are posted will be 
closed without comment.

However, I may very well make this enhancement in a future version of CSVfix.
