duplicate definition of /rfc/@consensus in DTD #19

As of [108}, the DTD has two declarations of the consensus attribute (*); one of them as #implied, one with a default of "yes".

Proposal: remove the default "yes" one (consistent with most of the other attributes).

(*) Shouldn't that be catched by a validating XML parser?
