Reverts helloSystem/docs#49

Cover was not rendered properly in Lector.app (our default EPUB reader).

Could definitely be that this needs to be fixed in Lector.app rather than the EPUB, but for now let's better ship no cover than our own documentation not rendering properly in our default EPUB reader app.
