Progression-relapse after the End of Treatment

1. Type  - "Please define if the relapse/progression was local, distant or combined."

In the model the element type (see https://build.fhir.org/ig/hl7-eu/pcsp/StructureDefinition-RelapseAfterFLT-definitions.html#RelapseAfterFLT.type) is used to distinguish relapse and progression , while "local, distant or combined" is conveyed by the qualifier element

To do:

• add the the mapping for the qualifer;
• check if 89780004 | Combined (qualifier value) | 46053002 | Distal (qualifier value) | 40415009 | Proximal (qualifier value) | are the proper concepts
• verify if these concepts are in the used value set for qualifier

2. If distant, site of metastases

To do

• verify if a notes field might be enough or if a linked "metastasis" Condition resource should be used

• [x] add a new extension to cancer conditions
• [x] remove combined from the qualifier value set
• [x] use bodysite for metastases sites
• [x] update mapping
