Use LCRelationNavigator to get "from" and "to" parameters #49

When trying to convert LCIO collections generated by DDPlanarDigiProcessor, LCRelationImpl won't generate the "from" and "to" parameters for TrackerHitPlane and SimTrackerHit relation.
