Remove add_entities_when_ready #413

Might be an overcomplication leftover from the older days where I've implemented the groups by hand. Switching to adding them on startup. This should improve MA startup times.

Will test for a couple of weeks to see if there's any adverse consequences.

This PR is in draft pending manual testing. Automated tests are passing.
