Using Survey 123 on the desktop, I am not seeing the first photo of 2 for the Upstream-Cover issue photo. The second photo is visible.

Did you add a photo in the first record? If it doesn't require one, you can have a blank record. I've adjusted this in 1.2 though.

OK. That is unfortunate. In order to help trace the issue can you provide:

Windows version number
Survey123 version number
Record a video/screen recording of the issue so I can see the steps. You can just film it on your phone and send to me or use an app like ShareX.
