Add layout template for course materials #4

Create a layout template to use for the course materials used in courses taught by the chapter.

Use sara's http://www.gdidayton.com/classes/intro-html-css/ as a guide for what needs to go onto the page, but give it the same style (css and html format) as the rest of the website.
