Updates to metadata entry #196

Currently we have some phenotypes (such as Alzheimer's) under phenotypes, but other phenotypes are in their own separate category (such as PD or stroke). We may want to make that consistent.

We also need to remove age for the list of metadata now that it is it's own separate field.

Also, it is not clear that you can add multiple tags. Once you've added one, the next option to add a model pops up. We could add a button that says done entering metadata before showing the next step and/or we could add in the help box that pops up its possible to add more than one, or suggest adding more than one (if applicable).

@nickbianco We have not made these changes. I think we may still want to make them, but we could deprioritize if we don't think it's a pressing issue compared to what else is on our plate.

I just removed the high priority. We could move it to future release.
