jack-name, mp3 samplerate, and helpfile cleanup #25

These changes add the ability to change the name of the instance in the patchbay, and set the samplerate of mp3 output. The first is to more simply detect multiple instances of jack_capture, and the other is used in my community radio station to capture a low quality MP3 log, saving significant disk space.

Also, and somewhat less useful, is a helpfile cleanup because I was having trouble reading it.
