wk2 sept_epic: update fidelity surveys in redcap for 3 MTL live sessions #2733

For active VA blue sites, fidelity surveys need to be added and activated in Redcap in advance of sessions. Emily to add 2 session 6 surveys and a session 3 survey for the MTL live sessions occurring in wk 2 and wk 3 sept_epic.

Jenn tested these on 9/15, confirmed they were were working correctly, and deleted her test records.
