Error out early if duplicates are detected in the source files list #139

Duplicates in the source files list can cause problems for ms_add_gtests which scans source files, so detect this problem early in ms_add_executable.

This, along with maidsafe/MaidSafe-Vault#12 fixes the CDash build targets.
