I have used the this version for this Fall 2020 Ph.D. dissertation final submission, and I got a few requests for changes from Rackham OARD. The changes that I made are mostly for Table of Contents (TOC) such that each chapter head in TOC needs to be in capital letters. I also found an error for "\appendix", so I implemented a package and change the appendix chapter with upper case letters for correct TOC appearance.
