Add index on processed for merit_actions table #356

Added the index in the migration template and the dummy app. I think this should cover it

Coverage remained the same at 90.684% when pulling 36574ace928c9a613d94a10e10d5344cfbd82abd on adrienpoly:add-index-to-merit-actions into bc04e6d0feb695f4d881dfe082fa3368f67a5fb9 on merit-gem:master.
