Fix all remaining line ending problems

Fixes all remaining text file inconsistencies by the following steps
1. Delete .git/index
2. Execute git reset to force it to rescan the entire directory structure for changes, which includes changes against the text settings in .gitattributes
Author set to @zooba so it doesn't look like I took ownership of everything (I messed this up last time)

We've gone too far astray for this PR to be valuable (my fault for letting it sit too long).

AFAICT, the line ending should be okay right now.
