Update MailChimp form on MM portal #2088

Maria Cruz Lopez and Lisa Eichel have requested that I update the MailChimp form on the MM portal. The reason that this update is needed is because marketing would like to start segmenting leads into separate lists according to which MM program they're interested in.

I have attached the new updated form, it just needs to be pasted over the old MailChimp form code.

@ChrisLanfranco thanks! Can you please describe what are exactly the changes in the signup form and we go from there ?

Sorry Chris, I misunderstood what we need to do to change this -- it's not just cut and paste.

We need to know the names of the fields that you're adding, and their ids in Mailchimp.

There was one dropdown field added to the submission form, nothing else was changed. The new field goes between the field NAME and Tell us more ...
