Add ability to ingest a csv/xls/xlsx file #3

For the initial use case, a user has a bunch of documents (pdfs/doc/ppt) in the input folder. It would be useful to allow the user to ingest a table file -- csv, xls, xlsx-- where each row is treated as a document. An old version of Rhapsode had this capability. Let's add it back, but allow the user to drive this via the ui.

There's currently an invisible beta for this at /rhapsode/indexer...make it better and make it work.

I've been working on this a bit more, and the invisible beta is available from master: /rhapsode/indexer
