Model drug combinations #11

So far we have made direct links between a disease and drug and variant and drug. However, in some cases we have drug combinations rather than a single drug, and we may eventually get additional metadata related to a therapy, such as dosage.

Therefore, we need to update the modelling of drugs in the knowledge base. Will use this ticket to bounce around some ideas.
