Reorder chapters 3.8.9 #169

this incorporates the changes from https://github.com/music-encoding/guidelines/tree/revise-chapter-03 and https://github.com/music-encoding/guidelines/tree/revise-chapter-09, and fixes #82, fixes #87 and fixes #88. These chapters are definitely not perfect yet, but the order of things should be much clearer now. This work has seen input from many people, including, but not limited to @margrethesb, @KristinaRichts, @lpugin, @musicEnfanthen, @irmlindcapelle, @rettinghaus, @bwbohl. Thanks for that! The plan is to include the state we've reached with this in v4, and then do the necessary refinements in dev, so that they will ultimately be part of an upcoming v5. I'd be happy if one of the reviewers would have at least a very brief look into this, or give some other feedback over the weekend. I'd like to get this out of the way to deal with other PRs that require this to be resolved completely… Thanks in advance :-)
