Allow human-readable name for pages (or indexes) #65

Now the pages are appended with Visio page ID (Page.ID). It is not page index, to keep files on reordering (if you make external links to the exported page, they will stay valid even if you rename pages in the drawing or reorder them).

This is to optionally allow using page names or indexes instead if the above is not wanted (under settings).
