Multiple Repeated Medical Histories Are Permitted

Adding repeated medical histories to the app is currently permitted. It might be useful to prevent users from doing this, whether on accident or on purpose, to prevent duplicate medical histories on a single patient, which may affect the patient's overall medical records.
