Some tooling updates only.

Changes:

insert the latest html toc automatically
added a workaround for replacing illegal html characters (backticks in glossary are currently not interpreted in glossary as they should).
removed pandoc title for html generation as that prepends the title above the OASIS logo on the title page which is wrong
