Refactor namespaces #5

Refactor OOSvrBase into OOBase, and hide internal implementation whenever possible, perhaps OOBaseImpl?

Need to look at hiding Proactor implementation a bit better, perhaps a specialisation of Singleton<> for Proactors will help.

Most of the rest is moving slowly over...

Singleton specialisation is donw for Proactor, but still in a separate namespace
