Upgrade server OS to Ubuntu Xenial 16.04 #327

~I guess not really a priority but it does already include docker~

Update: Ubuntu Precise 12.04 is End of Life. The latest LTS release is Xenial 16.04.

@henare describing the upgrade process in chat:

For that we should get everything working locally with Ansible on the upgraded OS. When we're satisfied it's working then we should deploy a new machine. Set production to read-only. Use the built-in morph rake tasks to backup and then restore to the new machine. Switch over IP addresses to the new machine and then shutdown the old machine.
