It would be good to come up with a way to ensure openEQUELLA is handling files appropriately.

Verbose logging whenever attachments are touched - this would provide the necessary audit trail to 'prove' openEQUELLA handled attachments properly.
A rubbish-bin area that instead of a file being deleted, it is moved there and logged, and a system admin has to manually go in and empty it - in theory, this would ensure you could always recover a file from openEQUELLA.
Detail / enhance known ways to monitor openEQUELLA file handling

CDM group thoughts:

Consider the 'Trash' folder, maybe making it more persistent.
For the rubbish-bin thought, you'd want an audit-trail to know where the files came from
Might want a scheduled task (configurable) to clear out older rubbish-bin content
It'd be a lot of effort to create a more transactional methodology, but that would eliminate some of the race conditions that can cause these concerns.
Consider a background task to check if there are any missing attachments (ie convert ping-{open}equella).
