Updates to Lab Test

• [x] For "test" could we just have 2 drop down options "Sputum culture" and "Atypical PCR"
• [x] Could we have 3 boxes for antimicrobial testing instead of the current one: "Antimicrobials Susceptible, Antimicrobials Intermediate and Antimicrobials Resistant"
• [x] Instead of a single checkbox of "Biobanked" could we have two checkboxes "Sweep Biobanked" and "Organism Biobanked"
