"Choose a block" - lumps blocks from different streets together on disambiguation page #256

For example: http://columbia.openblock.org/search/?q=101+N+4th+St It lists blocks on 4th Ave, N 4th St, and S 4th St, but the title says only "Choose a block of 4th Ave".

Blocks should be grouped by street, with titles for each.
