Implement the client profile medical history #14

[x] There will be no additions to the medical history, and this should be pulled from the CHW app. The medical history should be the same in the all family profile, register profile, and all clients profile.
[x] The health facility app will pull the medical history from other modules (child, ANC, [PNC] (https://github.com/OpenSRP/opensrp-client-chw-anc/issues/76), malaria, FP (yet to be implemented))

QA Build Oct 18:

Child Profile: The medical views are not the same in the CHW app and the HF app CHW

@msschroeder The medical history is implemented on CHW in a way that it is displayed after 24 hrs that is after it has been synced. The issue of medical history not matching on both CHW and HF apps has also been fixed.

PNC looks great!
ANC is almost there. However, the dates are appearing as null on the HF side: HF
