https://github.com/pqrs-org/Karabiner-Elements/issues/3620#issuecomment-1765946996 I'm on sonoma 14.2.1 and Karabiner 14.13 and this solution fixed for me
Still doesn't work unfortunately. It's just like Karabiner is not installed at all. I had zero problems before Sonoma 14.2.1
all key modifiers shown, but totally ignored.
Same here. Worked yesterday. Updated to 14.2.1 (23C71) -> broken (v14.12)
I had to fully reprogram my keys and then realized that since I have switched right-command and right-option keys, it seems that they now take precedence over previous keymaps. I changed all my right-command keys into right-option keys, and I have my mapping back. If you also have right-command and right-option switched in the Simple Modifications section, try to use either key with your shortcuts to see whether this makes it fine.
After updating to Mac OS 14.2.1 (Karabiner 14.13.0) I had problems with CMD and ALT (option) modifiers being ignored, after a few minutes of experimentation I discovered that my mouse was detected twice and as a keyboard. Disabling modify events on both returned normal behaviour and things works fine now.
I have a single single modification (right_shift --> delete_forward) - After updating MAC OS to Sonoma 14.1.2 and Karabiner-Elements 14.13.0, my modifier no longer works. I tried the above and even uninstalled then reinstalled the pkg. Nothing has worked.

Thanks you man. This solves it also for me ...
Just upgraded to 14.2.1 (23C71) and Karabiner is loaded, all key modifiers shown, but totally ignored. Was fine with 14.2
