Fix page IDs (FORM_PAGE_ID) #151

IDs are not updated when copy/pasted, e.g.:
private static final String FORM_PAGE_ID = "projectPracticeTechniquesPage"; 
//$NON-NLS-1$   

is found twice

Page classes should match convention for names: 
CustomTypeAssociationType(plural)Page
